Types of Retainers We Provide

Removable Clear Retainers (Essix)

Clear retainers — sometimes called Essix retainers or vacuum-formed retainers — are the most commonly prescribed retainer at Bright Healthy Smiles. They are made from clear, thin plastic that fits precisely over the teeth, making them virtually invisible when worn.

Clear retainers are:

  • Comfortable and discreet — most people cannot tell they are being worn
  • Easy to remove for eating, drinking, and cleaning
  • Simple to clean with a toothbrush and water
  • Less likely to affect speech than wire retainers

Clear retainers do wear over time and need to be replaced periodically — typically every 1 to 3 years with regular wear, sooner if damaged or lost. We recommend keeping the most recent set of orthodontic records on file so replacement retainers can be made quickly if needed. When a family calls about a lost retainer for their child, our Manhattan team can typically fabricate a replacement from the digital scan on file — often before meaningful shifting occurs.

Types of Retainers We Provide

Fixed (Bonded) Retainers

Fixed retainers — also called bonded or permanent retainers — consist of a thin wire bonded directly to the back surfaces of the front teeth. They are not removable and provide continuous retention without any compliance requirement.

Fixed retainers are often recommended for the lower front teeth, which have the highest tendency to shift after treatment. They are also used for patients whose compliance with removable retainers is a concern, or in cases where the orthodontist anticipates a particularly high tendency for shifting.

Fixed retainers require some additional care with flossing — a floss threader or water flosser is needed to clean beneath the wire. We provide instructions at placement and are available for questions as patients adapt.

Hawley Retainers

Hawley retainers are the classic wire-and-acrylic retainer. They consist of a custom-fitted acrylic base that sits against the roof of the mouth or along the lower gum line, with a wire that wraps around the front teeth.

Hawley retainers are more durable than clear retainers and can be adjusted by the orthodontist if minor tooth movement occurs. They are a good option for patients who need a more robust appliance or for specific post-treatment situations where some minor adjustment may be needed during the retention phase.

Common Retainer Questions We Hear from Families

Answers to common questions families ask about retainers and orthodontic retention.

Call us as soon as possible. Time matters because teeth can begin shifting within days to weeks of consistent retainer non-wear, depending on how long the patient has been in retention.

If the loss happens shortly after treatment completion, replacement is the most important — this is when shifting risk is highest. If it happens years into retention, there may be slightly more flexibility, but replacement is still important. We keep records on file and can typically produce a replacement retainer efficiently.

Do not wait. Do not assume the teeth will stay put. Call us and we will help you figure out the fastest path to a replacement.

This is one of the most common situations we hear from families. Teenagers forget, get out of the habit, or decide on their own that they are ‘done’ with their retainer.

If the retainer still fits — meaning it goes in without significant force — resume wearing it immediately and contact us for a check-in appointment to evaluate any movement that may have occurred.

If the retainer no longer fits — meaning there is resistance or it will not seat properly — do not force it in. Call us. We will evaluate what has shifted, take a new record, and discuss your options. In some cases, a short period of refinement treatment may be needed to recover movement before a new retainer is made.

Full-time for the first several months after treatment, then nightly — indefinitely. The American Association of Orthodontists is direct about this: retention is lifelong because tooth drift is lifelong.

Clear retainers should be rinsed with cool water and brushed gently with a soft toothbrush and mild soap (not toothpaste, which is abrasive and can scratch the surface). Retainers should be stored in their case when not in the mouth — the most common way retainers are lost or damaged is being left on a lunch tray, left on a table, or placed in a pocket without protection.

Hawley retainers can be cleaned the same way. Fixed retainers require careful flossing beneath the wire at least once daily.

Teeth move throughout life in response to the natural forces of biting, chewing, and soft tissue pressure. A retainer counteracts this tendency and holds the teeth in their corrected position. Without retention, even beautifully aligned teeth will gradually shift. Retainers are not optional — they are the mechanism that preserves the result of treatment.

Call us immediately. Teeth can begin to shift quickly after retainer non-wear, especially in the first year or two after treatment. We keep records on file and will get a replacement made as quickly as possible. Do not wait weeks to address a lost retainer.

This is a very common challenge. We find it helps to have the conversation directly with the teenager at their appointment — sometimes hearing the explanation from the orthodontist carries more weight than hearing it from a parent. We can also discuss whether a fixed (bonded) retainer is appropriate as a backup for non-compliant patients.

Most patients transition to nighttime-only wear after the first 6 to 12 months of full-time retention. We monitor each patient individually and make this recommendation when the teeth have demonstrated stability. We will let you know — do not make this transition on your own without checking with us.

If the shifting is minimal and the retainer still fits, resumed consistent wear may help stabilize the teeth. If the retainer no longer fits or if there has been significant movement, new records and possibly a short course of refinement treatment may be needed.
