Early Detection with Advanced Technology

We cannot treat what we cannot see — and many early cavities are invisible to the naked eye. Bright Healthy Smiles uses CariVu technology, a transillumination device that passes harmless near-infrared light through the tooth to reveal decay in its earliest stages, including between teeth where X-rays sometimes miss it.

We also use low-radiation digital X-rays to minimize cumulative radiation exposure while maintaining diagnostic accuracy. With CariVu cavity detection, children at our NYC office benefit from earlier answers — detecting decay early, before it penetrates deep into the tooth, means we can intervene with the most conservative treatment option rather than a more involved one.

Monitoring Before Treating

Not every early lesion requires immediate restoration. When decay is detected in the outer layers of the enamel and has not yet progressed to the dentin, we may recommend a watch-and-wait approach combined with enhanced preventive protocols.

Fluoride treatments, dietary modifications, improved home care, and, where appropriate, silver diamine fluoride can arrest early decay progression. Many early lesions can be remineralized — essentially reversed — without any restorative intervention at all. We document these lesions at every appointment, track their progression, and restore only when the evidence indicates the lesion is advancing despite preventive efforts.

Conservative Cavity Treatment

When a filling is needed, we use composite resin (tooth-colored material) that bonds directly to the tooth. This bonding mechanism allows us to remove less healthy tooth structure than was required with older silver amalgam fillings, which relied on mechanical retention and required a larger preparation.

For very small lesions, we sometimes use a technique called sealant restoration or preventive resin restoration — a hybrid approach that removes only the decayed portion of a fissure and seals the remaining grooves in the same appointment. The result is a treatment that is less invasive than a traditional filling while fully addressing the clinical problem.

Silver Diamine Fluoride (SDF)

Silver diamine fluoride is a topical liquid that can arrest active dental decay without any drilling or injection. It is not a permanent solution for large cavities, but for early cavities in very young children — particularly children who cannot yet tolerate conventional treatment — SDF provides an effective way to halt progression and buy time until the child is developmentally ready for restorative care.

SDF is also used for high-risk patients as an additional layer of cavity prevention. It has a strong safety record and is increasingly recommended in the pediatric dentistry literature as a first-line option for managing early childhood caries — a hallmark of biological pediatric dentistry in NYC done responsibly.

Preservation of Primary Teeth

Baby teeth matter. They hold space for permanent teeth, support normal speech development, enable proper chewing function, and contribute to a child's confidence and social development. When a primary tooth can be saved, we save it — even if the treatment is more complex than extraction.

A pulpotomy followed by a crown is almost always preferable to extraction followed by a space maintainer. Keeping the natural tooth in place provides better space holding, better function, and a simpler clinical situation for the erupting permanent tooth.
