Minimally Invasive Pediatric Dentistry

Silver Diamine Fluoride-SMART (silver modified atraumatic restorative technique). The less is more in pediatric dentistry. Silver Diamine Fluoride (SDF) is a revolutionary approach to combating caries in a non-invasive manner. Following a widely publicized 2016 feature in the New York Times, there has been tremendous interest surrounding the use of SDF from patients and dentists.  In truth, silver diamine has been used for years in Australia and Japan but the FDA only recently allowed the use in the US.  This liquid mainly contains silver as a potent antibacterial/ bactericidal/ desensitizing  agent and fluoride as a remineralization agent. It is a colorless agent that is painted or brushed on  the affected area of the tooth. This is an effective method to kill cavity-causing bacteria without having to use the drill and fill technique. However it needs to be used as part of a comprehensive caries management program. Young patients can greatly benefit from a minimally invasive approach. But it is important to remember that, at times, more treatment might be necessary later. The areas of the tooth treated with silver diamine can be covered with a glass ionomer, a white, fluoride-releasing restorative material to mask the gray discoloration of the tooth and further protect the area. While silver diamine is a very good solution for young children and special needs children it is important to remember that no one  approach fits all cases.
